Thank you for your interest in becoming a vendor at Overdose Awareness Day hosted by Big Fish Ministries. This event will be held rain or shine.

Vendor booths are first come, first served and include a 10x10 space (approximately one parking space).

Please note that space is limited and restrictions may be placed on the types of merchandise and vendors permitted. You will be contacted via email regarding the status of your application.

Vendor Agreement

I agree that my name, image and likeness as well as any photographs of me or my work and/or booth may be used for publicity purposes. I agree that I will not assign all or part of my space to any other person or organization without the express permission of Big Fish Ministries. I agree that my space will be used for the sole purpose of promoting my products and services as listed on this application.
